Post both pages together as a two-image post, or on LinkedIn add the PDF to your post as a document.

For California ACH Week, we're excited to spotlight Redwood City Together in San Mateo County and its work to create opportunities for young people through caring connections and community partnerships.

Through its Community Collaborative for Children's Success, Redwood City Together partnered with 12 community organizations to provide summer employment for 37 young people. The team also developed guides, provided coaching, and conducted check-ins with youth and employers to help ensure that young people have supportive experiences.

Redwood City Together brings residents together through community Cafecitos focused on mental health, social media, safe and caring adults, and other topics important to families.

These efforts reflect the partnership's approach to violence prevention: investing in young people, strengthening relationships, and building community connections before crises arise.

As Redwood City Together puts it: "Caring adults reside here for youth."

A California ACH Week highlight from Mission Promise Neighborhood (MPN): In October of 2025, MPN hosted its 11th Annual Education Forum, centered on Family Wellness. Partners led a panel and facilitated deep-dive sessions designed to equip families with practical tools for health and resilience:

Good Samaritan Family Resource Center: Explored "Family Dynamics Through the Life Stages," helping families navigate the evolving needs of their children.

Felton Institute: Hosted "Self-Care and Stress Management," providing essential mental wellness techniques for parents and caregivers.

Mission Graduates: Led vital conversations on "Alcohol, Tobacco, and Drugs," focusing on how parents can maintain open, proactive communication with their youth.

MEDA: Elevated the next generation through the "Youth for Health" (Jóvenes por la Salud) workshop, where youth shared their own participatory research on youth mental health.

A parent shared, “I learned about how to manage depression and how to talk to our kids about it.”

For California ACH Week, we’re highlighting Humboldt Community Health Trust. Working with community partners, the Trust built North Coast Care Connect, a shared system for referrals and care coordination. Forty-five organizations now use it. When one refers someone, it can see whether the person was connected to services and follow up if needed. Completed referrals have grown from fewer than 40 to more than 150.

The Trust also convened treatment providers, community navigators and the regional health plan to improve coordination of substance-use care. After two meetings, the health plan asked to become a formal partner and began considering how the approach could work in other counties.

Accountable Communities for Health bring local partners together on problems no single organization can solve alone. Learn more at www.californiaachweek.org

In our 2026: 10 Years of Impact report, we highlighted one systems transformation led by each Accountable Community for Health (ACH) in our statewide network. Taken together, these 36 Transformations illustrate the diversity and impact of ACH work. For California ACH Week, we are celebrating these efforts and invite you to explore the six transformations shared here.

- Compton Healthy organized big community health events, like the "Compton Walk for a Cure" and the "5K for $5K Scholarship Run," that brought out hundreds of residents.

- Contra Costa Together helped the county improve how care providers work together and protect patients who are in jail and undocumented residents at county facilities.

- When COVID-19 hit, East San José PEACE Partnership got $600,000 in emergency aid for residents who were left out of other financial help, and pushed for protections against eviction.

- Eden Area Communities Collaborative helped the county to create an “Office of Unincorporated Services” and include communities outside city limits in its plan for spending Measure W money.

- Fresno Community Health Improvement Partnership expanded the number of community health workers across Fresno County by creating a university training program and launching a hub that connects residents with service providers.

- Health Equity Rising worked with residents from four neighborhoods to create an action plan with the county to improve health equity.
